OUTPATIENT SERVICES
CASE MANAGER
This position reports to the Program Manager. He/she provides
counseling to clients and documents all contacts in the client chart.
INTAKE SPECIALIST
To provide direct services to clients including intake assessment, follow up and
referral services. Will work under the direct supervision of the
Program Manager.
PROGRAM AIDE
Program Aide provides clerical support for the outpatient services and
day care habilitative services. Reports to the Program Manager.
PROGRAM MANAGER
The Program Manager reports directly to the Director of Operations and
is responsible for keeping him/her abreast of the status of all
programs. The Program Manager is responsible for ensuring optimal
quality of services being delivered to clientele. The Program Manager
is furthermore responsible for assisting the Director of Operations in
the auditing of the client charts. The Program Manager is accountable
for the day-to-day supervision of direct service staff.
VAN DRIVER
Van Driver coordinates transportation services, and transports clients to and
from the program. This position reports to the Program Manager.
